Dr. Anthony Mair

Sidestepping Surgery with Radical Realism

BIOGRAPHY
Dr. Anthony Mair received his dental degree from McGill University in 1987 and his Master of Clinical Dentistry in Orthodontics from University of Western Ontario in 1990. Since then, he has been in private practice in Toronto. Dr Mair is an Adjunct Professor in the Graduate Orthodontics department at Western University and a Clinical Instructor in Graduate Orthodontics at University of Toronto. He is a Past President of the Toronto Orthodontic Club and the Ontario Association of Orthodontists, and has served on the Scientific Content Committee of the Canadian Association of Orthodontists since 2015. Dr Mair is currently serving his second term as a Councillor at the RCDSO. He has a particular interest in Biomechanics, Treatment Efficiency and Non-surgical management of complex cases.

BIOGRAPHY

Dr. Benjamin Pliska is a graduate of the University of Western Ontario School of Dentistry, and obtained his Certificate in Orthodontics and Master’s Degree in Dentistry from the University of Minnesota. He is an Associate Professor of the University of British Columbia Faculty of Dentistry, an Orthodontic Consultant at B.C. Children’s Hospital and maintains a private practice in Vancouver as a certified specialist in Orthodontics. Dr. Pliska’s research interests include craniofacial imaging and sleep medicine.

 

Dr. Bart Iwasiuk

“You Can’t do that with Invisalign”

There is a common misconception that as orthodontics, we should look for “candidates” that are appropriate for aligner therapy.   Although this may have been an appropriate approach in very early years of Invisalign Aligners being available for our use, over the decades not only has the appliance itself greatly improved, but our understand of bio-mechanics and bio-materials has improved dramatically.  To that end, essentially all patients can be successfully treated using Invisalign Aligners.   This presentation will focus on trials and tribulations encountered over the years to allow for now routine use of Invisalign Aligners in treating patients requiring bicuspids extractions.   From early misses to today’s successes, see how you too can incorporate bicuspid extractions into your Invisalign tool box.

BIOGRAPHY

Dr. Bart Iwasiuk has been in private practice since 2002 and using Invisalign since his residency training. He achieved Elite Provider status in 2012, and continues to push the limits of what can be accomplished using  Invisalign aligners. He received his DDS from the University of Toronto and his Orthodontic Certification from the University of Rochester. His Interest in multidisciplinary dentistry stems from spending two years at the University of Washington and being influenced by the Seattle Study Club. He is an Associate Professor at the University of Rochester, as well as a personal instructor on YourOrthoCoach.com. He is an International Keynote Speaker, Research Consultant and a member of the Key Opinion Leaders for Align Technology, Inc.. With advancements of Aligner products aimed at growing patients he is also a Member of the Global Growing Patient Speaker Group and the North American Mandibular Advancement Clinical Expert Panel. He is an Invisalign Diamond Provider practicing in the Greater Toronto area.

 

Dr. Sylvain Chamberland

Literature review on palatal expansion and evolution of palatal distractors from dental anchorage to bone anchorage in non-growing patients and review of the surgical indication (SARPE, DOME).

BIOGRAPHY
Dr. Sylvain Chamberland received his DMD degree from Laval University in 1983, a Certificate in Orthodontics from University of Montreal in 1990 and a Master degree in dental science in 2008 from University Laval. He has lectured in several graduate programs in orthodontics and scientific meetings in the US, Canada and Europe, and has published articles about SARPE, genioplasty, PCR/ICR and combined treatment SARPE & MSDO, Biomechanics of open bite treatment and T-loop retraction spring. He conducts an active clinical practice, is a Diplomate of the American Board of Orthodontics, a Fellow of the RCDC and a member of the EHASO.

Interdisciplinary Collaboration: An Approach to Optimize Treatment Outcomes for the Complex Orthodontic Patient

Program Description
Achieving esthetic excellence in the debilitated dental patient is one of the most challenging tasks an orthodontist will undertake. These patients often require dental treatment to correct specific anterior esthetic and functional discrepancies. Unfortunately, many also present with treatment challenges such as missing or impacted teeth, severely worn incisors, esthetic concerns, and/or loss of periodontal attachment. Therefore, planning treatment is difficult and requires coordination of care between multiple clinicians to achieve an optimal functional and esthetic outcome. This presentation will highlight the orthodontist’s role on the team in developing treatment plans that optimize esthetics, while still satisfying specific functional requirements for these patients. It is designed to provide a systematic approach to diagnosis and treatment of various esthetic dilemmas that we encounter in our practices every day.

Learning Objectives
After hearing this presentation, attendees should be able to:
• utilize the interdisciplinary team when diagnosing and treating ortho/perio/restorative problems
• appropriately sequence treatment when managing orthodontic patients with an interdisciplinary approach
• identify the value of orthodontic treatment in order to facilitate the most conservative and esthetic restorative treatment

Biography
Dr. Kokich Jr. earned his dental degree from Tufts University School of Dental Medicine in 1996 and completed a general practice residency at the University of Washington Medical Center. He remained at the University of Washington for his orthodontic training where he obtained his Masters in Orthodontics in 1999. He has continued his involvement at the University as a guest lecturer in the Department of Orthodontics, while also maintaining a private practice in Tacoma, Washington.

Dr. Kokich is a diplomate of the American Board of Orthodontists and a member of the Angle Society and the American Academy of Esthetic Dentistry. He was honored to receive the Charles L. Pincus Research Award for Clinical Research from the American Academy of Esthetic Dentistry. He has written or co-written four book chapters as well as numerous scientific and review articles, and his current research and publications focus on esthetic, interdisciplinary dentistry.

Dr. Kokich Jr. has given over 250 lectures nationally and internationally, sharing his interest in interdisciplinary dentistry and dental esthetics, emphasizing comprehensive treatment planning and the importance of coordinating orthodontic, periodontal, and restorative care.

Dr. Michael Miloro

Biography
Dr. Michael Miloro is the Daniel M. Laskin Professor and Head of Oral and Maxillofacial Surgery at the University of Illinois at Chicago College of Dentistry. He attended the University of Rochester in NY, earning a B.S. degree in Neuroscience. He attended dental school at Tufts University in Boston, and completed medical school and residency training in General Surgery and Oral and Maxillofacial Surgery at the University of Pennsylvania in Philadelphia.

Dr. Miloro has held academic positions at The Ohio State University, University of Maryland, and University of Nebraska. Dr. Miloro is a Diplomate and past-Examiner for the American Board of Oral and Maxillofacial Surgery, a Faculty Fellow of the American Association of Oral and Maxillofacial Surgeons, a Fellow of the International Association of Oral and Maxillofacial Surgeons, and a Fellow of the American College of Surgeons.

Dr. Miloro has received numerous awards including the IAOMS Distinguished Service Award, the AAOMS Donald B. Osbon Award, the AAOMS Daniel M. Laskin Award, and the AAOMS Clinical Research Award.

Dr. Miloro has major clinical and research interests in orthognathic surgery, TMJ surgery, and trigeminal nerve injuries. Dr. Miloro has lectured nationally and internationally, and has published extensively in the medical and dental literature with over 200 peer-reviewed publications. Dr. Miloro is Editor of three major textbooks including Peterson’s Principles of Oral and Maxillofacial Surgery, Complications in Oral and Maxillofacial Surgery, and Trigeminal Nerve Injuries.

Biography
Dr. Caminiti is originally from Montreal and went to McGill University for undergraduate studies in Anatomy and Dental School. He was in the Canadian Armed Forces and served in the United Nations. At the University of Toronto, he completed his residency in Oral and Maxillofacial Surgery; a Fellowship in Surgical Education in the division of Surgery; and he was the Walter Lorenz Fellow in Orthognathic Surgery.

• Associate Professor, Head and Program Director of Oral and Maxillofacial Surgery at the University of Toronto
• Graduate OMFS Program Director at Mount Sinai Hospital
• Head of OMFS at Humber River Hospital
• Surgical Director at the Cleft Lip Palate Program at Holland Bloorview Kids Rehabilitation Hospital
• Chef de Mission, Face the Future Foundation; Addis Ababa

Dr. Caminiti’s practice is limited to the management of facial deformities and his research focus is on the development and assessment of surgical skills.

Biography
Dr. Markiewicz is Professor and Chair of the Department of Oral and Maxillofacial Surgery at the University of Buffalo, and adjunct Professor in the Departments of Neurosurgery and Surgery at the Jacob’s School of Medicine and Biomedical Science. He is an attending Head and Neck and Reconstructive Surgeon at Roswell Park Cancer Center, and is Co-director of the Cleft and Craniofacial Team at the Craniofacial Center of Western New York.

Dr. Markiewicz earned his Dental Degree at the University at Buffalo, and Medical Degree at Oregon Health and Science University where he also completed his General Surgery and Oral and Maxillofacial Surgery training. He obtained a Master’s of Public Health from Harvard University with a concentration in Biostatistics and Epidemiology, and completed a Fellowship in Clinical Investigation at The Massachusetts General Hospital, Boston, Massachusetts.

Dr. Markiewicz completed a fellowship in Pediatric Cleft and Craniomaxillofacial Surgery at the Arnold Palmer Hospital for Children in Orlando, Florida. To complete his surgical training, he completed a fellowship in Head and Neck Oncologic and Microvascular Reconstructive Surgery in the Division of Head and Neck Surgery at the University of Florida, College of Medicine, in Jacksonville, Florida.

Dr. Markiewicz’s clinical and research interests are dedicated to the management of Cleft and Craniofacial anomalies in children and adults, the treatment of Head and Neck Cancer, and the reconstruction of congenital and acquired defects in children and adults.

 

Dr. Brian Farrell

Dr. Brian Farrell

Dr. Brian Farrell received his Doctor of Dental Surgery from the University of Iowa College of Dentistry and completed medical school in conjunction with oral and maxillofacial surgery training at Louisiana State University in New Orleans at Charity Hospital.

Dr. Farrell practices with Carolinas Center for Oral and Maxillofacial Surgery in Charlotte, North Carolina and maintains a position as an Assistant Clinical Professor with Louisiana State University assisting in resident training. He serves as director of the fellowship program offered through Carolinas Center for Oral and Facial Surgery covering the broad scope of oral and maxillofacial surgery.

Dr. Farrell is a diplomate and served as a board examiner for American Board of Board of Oral and Maxillofacial Surgery and has authored and edited multiple clinical articles, chapters and textbooks and lectured nationally and internationally covering dentofacial deformities, digital surgical planning, and delivery of orthognathic surgery in an office setting with same day discharge.
